You cannot predict every emergency, but you can make the important decisions beforehand.
Complete the sections that are relevant to your household. Discuss the plan with everyone involved and keep a printed or electronic copy somewhere secure and accessible.
Your information remains private. This form does not send your answers to WHR or save them on this website. Until you print or save the plan, the information exists only in this open browser page. Avoid completing it on a public device, and store finished copies securely.
Immediate danger: call 999. If escaping a house fire or another immediate threat, leave at once—never delay to collect this plan, a Grab Bag or possessions.
